Application Form for Changing Thesis Title (Word or ODT file)

If you need to change your master’s thesis title, please complete the Master’s Thesis Title Change Application Form and submit it to your department/institute.

The content of application documents for thesis oral examination should be mainly written on a computer (handwritten documents are not accepted) and submitted to the department office.

【Application Procedures for Degree Examination】

Please prepare all required application documents in full. No supplementary documents (such as English proficiency certificates) will be accepted afterward. Applications with missing documents will not be processed.

【Documents to Submit to the Department Office After the Examination】

All documents must be prepared before the oral examination. On the day of the exam, each committee member must sign four copies of the documents.

Submit the documents：

1. The Master’s Thesis Review Application Form and its attached Committee Member List (Word or ODT file) consist of two documents. Both documents must be submitted in hard copy to the department office, and the electronic version (Word file without the advisor’s and department chair’s signatures) should be emailed to paper@mail.mcut.edu.tw .

2. English version：Academic Ethics Statement（Word檔、ODT檔）- One document.

3. Proof of English Proficiency - One document.

4. Certificate of completion of at least 6 hours of academic research ethics education - One document.

5. Plagiarism detection report ("Exclude quotes","Exclude bibliography"and "Exclude matches" must be turned off. Please attach your thesis, double-sided printed.) - One document.

6. SDGs Indicators for Thesis Research (Excel or ODS file)–One document , and SDGs Indicators Introduction (PDF).

to the department office. ***

I. Submit the documents：

1. Master’s Thesis Review (Oral Examination Evaluation Form) (Word or ODT file).

    Evaluation Form (1) * Three documents + Evaluation Form (2) * One document.

2. English version：MCUT Relevance of Thesis/Dissertation to Major Checklist（Word、ODT）- One document (Use the oral exam date).

3. Approval Document of the Examination Committee (copy) (Library Link)–One document (Use the oral exam date).

II. Please deliver the 6 copies of the documents to the department office.
